Family and carer experiences of mental health care and treatment for people from ethnically diverse backgrounds

This online questionnaire can be completed by carers of individuals from an ethnic group, who have lived experience of mental health difficulties and illness. We are keen to hear about your experience of caring for those accessing and receiving support from mental health services.

This survey should take around 20 minutes to complete.

The closing date for responses is Friday 7 May 2021.

National findings will be presented in our report, due for publication in September 2021.

If you have already met with a practitioner from the Commission in person to go through a questionnaire, then please do not complete this survey.

Information gathered from these surveys will be summarised in a final report written by the Mental Welfare Commission. 

Individual responses you give will not be used to identify you and will remain anonymous and confidential. When we use quotes in the final report we will only identify a service if the comment is positive. Any quotes used are anonymous. Negative comments may be used in the report but will not identify the name of any service. 

At the end of the survey you will be invited to add your email address. This is not compulsory, but recommended as you will receive a confirmation of your submission (and a copy of your responses). Also, this code will allow us to locate your response if you change your mind after submitting the survey. We will not use your email address in any other way.
