Overview

This online questionnaire can be completed by individuals who have an eating disorder.

This survey should take around 20 minutes to complete.

The closing date for responses is Sunday 8 March 2020.

National findings will be presented in our themed visit report, due for publication in 2020.

Why We Are Consulting

The aim of this questionnaire is to seek the views of individuals with an eating disorder.

If you have already met with a practitioner from the Commission in person to go through a questionnaire, then please do not complete this survey.

Information gathered from these surveys will be summarised in a final report written by the Mental Welfare Commission. 

We need to know your home health board and we ask for your age and gender, but we do not need to know your name. 

When we use quotes in the final report we will only identify a service if the comment is positive. Any quotes used are anonoymous.

Negative comments may be used in the report but will not identify the name of any service. 

At the end of the survey you will be invited to add your email address. This is not compulsory, but recommended as you will receive a confirmation of your submission (and a copy of your responses). Also, this code will allow us to locate your response if you change your mind after submitting the survey. We will not use your email address in any other way.

Online responses will be populated in our secure system.
